The challenge

Hackensack University Medical Center (UMC) a 775-bed non-profit teaching and research hospital in Bergen County, New Jersey sought to develop a program to improve post-discharge outcomes.

The solution

Hackensack UMC partnered with Cardinal Health to develop a leading-edge bedside prescription concierge service for patients leaving the hospital. Patients now leave the hospital with medications in-hand and can take their medications without stops, delays or interruptions.
