Ask me about:  care coordination, discharge management, strategies for reducing hospital readmissions, innovation in healthcare

Heather O’Sullivan’s experience spans nearly two decades in clinical, operational and leadership roles serving hospitals and health systems, health plans and home healthcare environments. As a nurse practitioner, she has deep clinical expertise across the care continuum from admission to discharge, as well as alternate sites of care. She brings this deep expertise to her role as chief clinical officer of naviHealth, a Cardinal Health company specializing in post-acute care management, where she leads their Health Services group overseeing the organization’s centralized clinical design and operations.

Prior to joining naviHealth, she was vice president of Clinical Operations for Cardinal Health and worked with clinical leaders of large health systems to analyze their clinical protocols and identify areas with the highest levels of variability. With this knowledge, she developed customized solutions to help health systems improve patient outcomes and reduce the cost of care.

She is a member of the Healthcare Businesswomen’s Association, Massachusetts Coalition of Nurse Practitioners and Boston-based Women in Healthcare Management.
