Essential to Wellness grants are specifically designed to improve the health and wellness in communities where Cardinal Health has a significant employee presence.
You will be required to create an account in order to access the letter of intent application. The username and password will be requested each time you access the application. If your organization is invited to submit a full proposal, you will be asked for this information and not required to complete the contact portion of the application a second time. In addition to submitting the online application, all organizations requesting funding must complete the budget template worksheet located on this page. Please upload this completed worksheet before submitting.
