A Pharmacist-driven Anticoagulation Clinic will be established at Fairmont General Hospital (FGH) in Fairmont, WV. The clinic will focus on improving anticoagulation for patients referred by physicians as outpatients and all inpatients on anticoagulation medication at FGH. The use of standardized practices that include patient involvement, education, and routine management with monitoring can reduce the risk of adverse drug events associated with the use of anticoagulant medications. The new service will help reduce the risk of patients developing major hemorrhage or thromboembolism, thus reducing adverse patient outcomes and improving overall patient care. The service will also assist FGH in meeting the 2008 National Patient Safety Goal 3E: Reduce the likelihood of patient harm associated with the use of anticoagulation therapy. Improvement in continuity of care from the inpatient setting to the outpatient setting is also foreseen since the same staff will be providing anticoagulation services for both. The new service will also help expand FGH's commitment to providing superior care throughout the community.
