Pharmacy-Based Coumadin Monitoring
Mackinac Straits Hospital is a Critical Access Hospital located between Lake Michigan and Lake Huron in Michigan's Upper Peninsula. Our program promotes patient safety by enabling our Pharmacist to monitor our Acute Care patients and Long Term Care residents. We have one physician that cares for all of our Long Term Care residents as well as our Acute Care patients. Our goal is to lessen his workload and improve patient care by moving Coumadin therapy monitoring to our Pharmacy department. We expect that this will enable us to monitor patient dosing carefully and drastically reduce medicine errors. The grant will help us fund the Pharmacist&rss certification and will assist us in the purchase of computer software to track Coumadin levels and dosing.
