The overall goal of this project is to decrease adverse drug reaction, simplify medication regimens, increase cost effectiveness through the use of generic medication, and promote increased patient knowledge about personal management of therapeutic medications. Successful achievements of these objectives would improve patient safety, organizational clinical measured goals and community awareness of appropriate medication use. MeritCare Health System intends to launch a community public education campaign in conjunction with the in-house program that focuses on personal medication knowledge. The project will specifically target high risk patients - those with multiple medications and chronic illnesses. The project target is safe use of medications in patients with chronic disease.
